Parent Permission Forms

The posting and showing of information online or any other way requires consent from parents or legal guardians when dealing with children under 18 years of age. This consent must be fully informed, freely given, current and specific as to who the information will be presented to!  Schools require signed authority for any work, films, images or information posted online. There are privacy laws for each state and different policies from each State and Territory Education Departments.   But whatever the laws and rules are - the issue is one of consent, in particular, parental or legal guardian permission is required when students are under 18 years of age. Below under supporting material is a generic parental permission form. You can add or delete text to suit your own requirements. You can check with your local Education Department for their recommended forms.
